• Critical Access hospital, 25 beds
• Call Required: Yes
• Call Ratio: 1:4 during the week. 1-5 weekend (every 5th weekend). Call requires OB
• Call starts at 3 PM
• Case Mix: OB (less than 400/year), GS, Ortho(lots of ortho), Endo, POD, Blocks, scopes.
• Trauma Level: Level 4 designation
• Patient Demographic/Ages: Some healthy peds but no babies, mostly adults.
• # of ORs: 3 OR
• Support Staff: Traditional OR support staff
• Supervision and Ratio:  CRNA staffing model
• Active License: Active NM, CSR, and DEA
• Credentialing Time Frame: 45 days
• Certifications Required: PALS, BLS, ACLS
