Title: Home Health Speech-Language Pathologist (SLP)
Location: Medford, OR and Surrounding Areas
Duration:13 weeks
Shift: Day shift, 8 hours (08:00 AM – 04:30 PM)
Job Responsibilities:
Provide skilled speech-language pathology services in a home health care setting.
Manage a patient caseload with a typical ratio of 1:1, visiting up to 6 patients per day.
Perform evaluations, assessments, treatment planning, and discharge processes.
Use clinical tools and assessments such as ALPS, BAE, BIMS, CADL, CELF, NOMS, WAB, and others.
Utilize adaptive and augmentative communication devices as needed.
Provide patient and family education to support therapy goals.
Chart and document care using EPIC and OASIS systems.
Collaborate with interdisciplinary teams including physical therapy, social services, and interpretation services.
Maintain compliance with documentation and billing requirements.
Required Skills and Qualifications:
Minimum 1 year of skilled Home Health experience as a Speech-Language Pathologist.
Current state license and BLS certification required.
Experience with patient evaluation tools and adaptive communication devices preferred.
Knowledge of dysphagia management and adult/pediatric/geriatric patient care.
Ability to work independently and manage time effectively in a home health setting.
Reliable transportation required due to travel between patient homes.
Proficiency with charting systems, preferably EPIC and OASIS.
Preferred Skills: Experience or training with specialized tools such as Acoustic Analysis, Nasoendoscopy, Videostroboscopy, Voice Prosthesis, and various aphasia batteries and cognitive performance scales. Familiarity with alternative communication devices like communication boards and memory aides. Prior knowledge of billing and documentation best practices in home health.
