Nationwide Therapy Group is seeking a travel nurse RN Case Management for a travel nursing job in Mission Hills, California.

Job Description & Requirements

Key Responsibilities:

• Assess patients and develop individualized plans of care using InterQual criteria (ED admissions).
• Coordinate care, manage DRG/plan of care requirements, and perform discharge planning.
• Arrange DME, SNF, and ambulance transport independently.
• Participate in family meetings and treatment plan development.
• Knowledge of special populations: drug/alcohol treatment, homelessness, mental health crises, advance directives, POLST, protective services.
• Document care in computerized charting system (Epic) and utilize Microsoft Office tools.

Requirements:

• Education: RN license required.
• Experience: Minimum 2 years as an Acute Care Hospital Case Manager.
• Certifications: Not explicitly required but standard RN certifications (BLS) expected.

• Travel Experience: Required.

   

Charting System: Epic required.
