We are seeking skilled and caring Licensed Vocational Nurses (LVNs) to join our growing team. You will be responsible for providing high-quality care to individuals in the comfort of their homes.

Responsibilities:
• Provide skilled nursing care to patients in their homes under the supervision of an RN and in compliance with physician orders.
• Administer medications and treatments as prescribed.

• Monitor and document patient conditions, vital signs, and progress.
• Collaborate with patients, families, and interdisciplinary team members to develop and maintain care plans.
• Educate patients and caregivers on disease processes, medication management, and ongoing care needs.
• Ensure accurate, timely, and thorough documentation in accordance with agency and regulatory standards.

Qualifications:
• Current and valid LVN license in the state of [State].
• Minimum 1 year of clinical nursing experience (home health experience preferred).
• Current CPR/BLS certification.
• Reliable transportation and valid driver’s license.
• Strong communication, critical thinking, and organizational skills.
