About the Role:

As a Physical Therapist in our Health Care Services team based in Corpus Christi, TX, you will play a critical role in improving patients' physical function and quality of life through personalized rehabilitation programs. You will assess, diagnose, and treat individuals with a variety of conditions including orthopedic injuries, sports-related injuries, and geriatric impairments. Your expertise will guide patients through manual therapy techniques and exercise regimens designed to restore mobility, reduce pain, and prevent further injury. Collaborating closely with physicians, occupational therapists, and other healthcare professionals, you will develop comprehensive care plans tailored to each patient's unique needs. Ultimately, your work will empower patients to regain independence and return to their daily activities with improved physical health and confidence.

As a Physical Therapist, you will:

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.

• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.

• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.

• Applies concepts of infection control/universal precautions in performing patient care

• Assure continuity of quality patient care delivered with appropriate documentation

• Provide effective communication to patient/family, team members and other health care professionals

• Promote agency philosophy and administrative policies

• Maintain and complete all documentation within company stated guidelines.

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.

• Valid Physical Therapy license

• 1 year of home health experience preferred

• CPR certification

• Valid Driver’s license

• Valid Auto Insurance

• HomeCare HomeBase experience preferred

Physical Demands

• Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.

• Must be able to lift, carry, pull, or push up to 100 lbs..
