Position Overview
The Licensed Vocational Nurse (LVN) provides skilled nursing care under the supervision of a Registered Nurse (RN). This includes direct and indirect care to meet clients’ curative, rehabilitative, palliative, or preventative needs. The LVN is responsible for reporting observations and care performed and is accountable for decisions based on clinical knowledge and training.
Essential Duties And Responsibilities
DUTIES
Provide skilled nursing care under the direct supervision of the Registered Nurse
Perform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care, as necessary, to meet the client’s needs
Perform direct and indirect care, then report care performed and client observations, through appropriate documentation
Responsible, and accountable, for making decisions that are based on the individual's nursing experience and educational preparation
RESPONSIBILITES
Assist the Registered Nurse in carrying out the plan of care
Assist the Registered Nurse in performing specialized procedures, as directed by the Registered Nurse, under physician's orders
Assist with the preparation, implementation, and continuing evaluation of the patient care plan
Recognize, and understand, the effects of social and economic problems upon patients and provide for the emotional and physical comfort and safety of patients
Help teach the patient the appropriate self-care techniques
Foster cooperative effort among personnel, by understanding the functions of other persons involved in patient care, and by active participation in team and staff conferences
Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on the job learning
Prepare, and submit, clinical and progress notes, no less often than, weekly (not to include admission)
Observe, record, and report, to the appropriate person, patient symptoms, reactions, and changes including:
The general physical and mental condition of patients and signs and symptoms, which may be indicative of changes in condition
Stresses in human relationships, between the patients and staff personnel, and between patients and their families and visitors
Assist with the rehabilitation of patients, according to the medical care plan, by encouraging the interests and special aptitudes of the patient and encouraging patients to help themselves using their own capabilities
Further assist in rehabilitation by reporting any abnormality in the patient’s range of motion, body mechanics, or body alignment.
Actively participate in nursing organizations, promote, and participate in in-service programs
TREATMENTS AND PROCEDURES
The Licensed Practical Nurse, with the education and proven clinical competence, may perform certain treatments/procedures, provided the Home Health Agency’s policies and the State’s Regulatory Acts allow their performance.
Only advanced LVNs, with documented skill in performing these functions, may:
Administer medications
Hang I.V.s premixed with medications
Hang hydrating fluid without medications
Start peripheral I.V.s
Flush peripheral lines with saline or heparin solutions
Inspect insertion sites, change dressings, and remove peripheral I.V. needles/catheters
Calculate and adjust flow rates
Check blood with a Registered Nurse
Job Conditions
Valid California LVN license
Reliable transportation to visit client homes and facilities
Ability to lift 50 lbs and perform essential physical duties
Proficiency in English (spoken and written)
The LVN is expected to work independently in the field and make real-time decisions within the scope of practice, without direct on-site supervision.
Must be proficient in using electronic medical records (EMR) systems and is required to document care in a timely, accurate, and compliant manner through mobile or tablet-based platforms.
Qualifications
Graduation from an accredited School of Practical Nursing
Must be licensed, in the state, as a Licensed Practical Nurse
One, or more years, of experience, in community/Home Health Agency setting, is preferred
Must have a criminal background check
Must have current CPR certification. Online certification is not accepted.
The ability to communicate well, both, verbally, and in writing, in English, is required.
Environment
Contact with clients under a wide variety of circumstances.
Care is provided in client’s living environment with varying situations, including private homes, independent or assisted living or in an institution such as a hospital or long-term care facility.
May be required to respond to emergency situations.
Travel required.
Position includes tasks that have the potential for exposure to blood/body fluids/tissues and other potentially infectious material such as body fluids/tissues.
Risk Of Exposure To Bloodborne Pathogens – High
Physical Requirements
Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull.
Must be able to lift up to 50 lbs.
Corrected visual and hearing acuity required.
Other Requirements
Must be able to provide two professional references
Must pass medical clearance and background check
Must be able to communicate clearly and effectively in verbal and written English
Work Hours
Work hours will vary depending on the client's needs. Work hours may include day, evening, weekend, or holiday shifts, based on client needs and the availability of the employee.
