Position Summary
The Case Manager I supports the program by providing case management services. The ultimate goal is to assist the patient with obtaining income, insurance, and housing. These services include assistance with co-occurring disorders (mental health and substance abuse) with the expectation of improving current functioning level and skills acquisition through assessment, engagement, assistance with access to treatment including medical care with a primary care provider, rehabilitation, and goals.
Required Qualifications
· High School Diploma or GED required. Lived experience sufficient to successfully perform the essential duties of case management which includes servicing populations of focus including but not limited to, unhoused, serious mental illness, substance use disorder and/or chronically ill patients. Associates Degree preferred.
· Ability to work collaboratively with others in a team environment, respecting and valuing the perspectives and experiences of others.
· Maintain and understand appropriate roles, and boundaries among patients, peers, and WellSpace partners/stakeholders.
· Able to function effectively in a fast paced environment.
· Demonstrate a commitment to the provision of services for the underserved.
· Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills.
· Computer (PC) literate in Microsoft Word, Excel, and Outlook email.
· Current CPR and First Aid certificate or willingness to be trained required within first (90) ninety days of employment.
· Ability to abide by standards of professional ethics and maintain HIPAA rules of confidentiality.
· Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
· Assist and support the Center/Department/Program to meet standards of High Reliability.
· Must have a valid driver’s license with an insurable driving record that meets the organization’s insurance policy.
· Must be willing to transport patients using personal vehicle.
Essential Responsibilities
· Meet with each patient individually to conduct an intake needs assessment.
· Manage a minimum of 50 patient caseload.
· Write and enter progress notes daily into EHR systems to maintain patient files.
· Incorporate a patient-focused approach to ensure the delivery of measurable outcomes.
· Perform 30% of duties in office and 70% of duties traveling to conduct in-person member visits.
· Must maintain a daily 80% utilization of billable patient services.
· Manage work schedule when meeting with members.
· Must be willing to transport patients to schedule appointments and community resources.
· Participate in care coordination meetings with various partners as deemed appropriate.
· Represent and/or advocate for and with patients in obtaining identification, housing, benefits services, and entitlements from governmental and private agencies and organizations.
· Coordinate linkage to various community resource programs and supports.
· Collaborate with the patient in the development of their individual SMART treatment plan.
· Rotate after hours on-call phone duties for a week at a time including weekends.
· Participate and complete all job related training, certification, renewals.
· Willingness to work flexbile work location, tasks may vary based on the needs of the program.
· Attend regular meetings with management and team.
