Full-time Primary Care Opening near Sparta, IL

Annashae is seeking Primary Care provider for a mental health facility near Sparta, IL. This is a full-time position & hours are Monday-Friday 8a-430p with no weekends, no call & no Holidays. We provide malpractice insurance & can credential quickly. 

Additional Duties include, but are not limited to:

• Provide treatment to a caseload of patients on the pavilion
• Evaluate assigned cases at intake
• Sign certificate of admission and testify in court
• Order and interpret lab tests
• Conduct initial physical exams(H&P) and makes diagnoses of patients
• Develop and implements treat plan
• Follow up exams and documents changes in progress notes
• Prescribe and direct administration of medicines
• Monitor effectiveness of medication

Requirements:

• Current unrestricted Illinois License 
• Board Certified/Eligible in Internal or Family Medicine
• Current Illinois Controlled Substance certificate
• Current unrestricted DEA certificate
• Mantoux skin test prior to starting work or documentation of negative Mantoux within last 90 days
• Clearance of required fingerprints and drug screen
