In this role, you will deliver individualized therapy, treatment planning, and case management services, working directly with clients to address mental health challenges and support their overall well-being. The clinician will collaborate with other team members to ensure services are effective, evidence-based, and in compliance with regulatory standards. This role also provides the opportunity to earn supervision hours for overall license eligibility. /p h2Responsibilities/h2 ul liProvide home-based individual therapy, family therapy, and case management services to children, adolescents, and families./li liDevelop and implement personalized treatment plans based on thorough clinical assessments./li liConduct risk assessments, safety planning, and provide crisis intervention as needed./li liMaintain accurate and timely documentation in the Electronic Health Record (EHR), including progress notes, treatment plans, and discharge summaries./li liEnsure services are client-centered, evidence-based, and meet medical necessity criteria./li liCollaborate with a multidisciplinary team, including Therapeutic Mentors, Therapeutic Training and Support (TTS) staff, and In-Home Therapists (IHT), to provide comprehensive care./li liAdhere to confidentiality standards, HIPAA regulations, and ethical guidelines in all client interactions./li liParticipate in clinical supervision, team meetings, and professional development opportunities./li liMeet productivity expectations by maintaining a caseload, which may include evening hours./li /ul h2Qualifications/h2 ul liMaster’s degree in Social Work, Psychology, or a related field that qualifies you for licensure in Massachusetts (e.g., LCSW, LICSW, or LMHC)./li liActive CANS certification required./li liExperience providing home-based or community-based mental health services preferred./li liProficiency in clinical assessments, treatment planning, and case management./li liStrong communication, organizational, and documentation skills./li liKnowledge of evidence-based therapeutic modalities and trauma-informed care./li liAbility to maintain professional boundaries, confidentiality, and ethical standards./li liValid driver’s license, reliable transportation, and ability to travel to client homes./li /ul
