Salary:

JOB SPECIFICATIONS:

  • Must be a graduate of an accredited college or university with a masters degree in social work or a related field (GSW) licensed by the state of Alabama, (LGSW).
  • Must have a least 1 year of experience in the field of Social Work to include Discharge Planning, Patient and Family Education, Formulation of Care Plans, and a history of Positive Interactions with referral resources.
  • Must be able to provide social services for patients ages 18 years old and up and manage stress appropriately, make decisions under pressure, manage anger/fear/hostility/violence of others appropriately, handle multiple priorities, work alone and work in areas that are confined and/or crowded.
  • Must possess the following skills: Organizational, Verbal, Interpersonal, Customer Relations, Analytical, and Grammar/Spelling, Read/Comprehend written instructions, Follow verbal instructions and other department specific skills.


POSITION RESPONSIBILITIES:

  • Must be able to cope with mental and emotional stress of the position and set the proper example.
  • Must function independently and have personal integrity and the ability to work effectively with personnel and agencies.
  • Must be in good health and demonstrate emotional stability.
  • Assess, distinguish, report and act upon the physical, social, developmental, and environmental needs of all patients; and the knowledge to provide age specific healthcare to patients as appropriate and necessary.


JOB DUTIES AND RESPONSIBILITIES:

  • Ensure patient dignity and respect patients values and operate within ethical standards.
  • Able to assess and reassess the needs of the patient.
  • Proper documentation of assessments and the ability to communicate needs of patients.
  • Complete psychosocial evaluations on all new admissions within 72 hours of admission.
  • Display good interviewing, diagnostic and report writing skills.
  • Complete progress notes, weekly Social Workers weekly summaries and assessments according to program guidelines.
  • Assist with development of individualized treatment plans for each patient.
  • Be an active member of the interdisciplinary team.
  • Conduct individual psychotherapy sessions in accordance with state regulatory

standards. Display good theoretical knowledge of psychopathology and good treatment

techniques with appropriate supervision.

  • Conduct family therapy sessions in accordance with state regulatory standards, display

good theoretical knowledge of family therapy principals and good treatment techniques

with appropriate supervision.

  • Conduct group therapy sessions in accordance with state regulatory standards,

displaying good theoretical knowledge of group therapy principles and good group

therapy techniques.

  • Engage in timely, efficient discharge planning, works in close conjunction with patient, family nursing staff, and attending physician.
  • Show good knowledge of referral placement process and available community resources.


PHYSICAL REQUIREMENTS:

  • Good verbal and written communication skills.
  • Visual/hearing ability sufficient to comprehend written/verbal communication.
  • Must be able to sit for 1hour/day and ambulate hour/day effectively.
  • Must be able to lift and move up to 30 pounds unassisted.
  • Must be able to hear alarms/telephone/tape recorder/normal speaking voice,

have good manual dexterity, good eye-hand coordination and have clarity of

vision.

  • Must be able to climb, bend, crouch, squat, crawl, kneel, balance self, and pull with force,
  • reach above head, reach above shoulders, and twist at waist.
  • Work sufficient hours to ensure effective performance. Is on 24-hour call to respond to
  • emergencies.
  • Provide ongoing education to patient and family as needed.
  • Conduct educational group sessions, display good theoretical knowledge of subject
  • matter and effective communication skills in presenting material.
  • Communicate effectively with families, agencies, conveying a positive professional
  • image.
  • May be a member of the Referral Development Committee.
  • Communicate results and trends to supervisor.
  • Be actively involved in measuring patient performance.
  • Provide feedback to supervisor on an ongoing basis in regard to concerns, improvement
  • and changes.
  • Identify areas needing improvement and utilizes facility PI Plan.
  • Actively participate in department processes as required.
