We are searching for a talented Community Health Worker III
Hours: 40hrs
Shift: Day shift; 8:00am - 4:30pm
Location: Southcoast Business Center - Fairhaven, MA; Position will follow a hybrid schedule

**Available to regular status employees who are scheduled to work a minimum of 24 hours.

- Associates degree or higher plus Community Health Worker certification and have two job-related certifications including but not limited to: Alcohol and Drug, SHINE/CAC or SHIP certified, Peer Recovery Coach, Health/Life Coach, Mental Health Counselling, Behavioral Health, Integrative Behavioral Health, Licensed Addiction Counsellor, National Certified Addiction Counsellor Level I and II Certifications or Interpreter Certification(s).
- Six or more years experience in community-based outreach.
- Proven ability for independent decision making and problem solving while performing effectively as part of a multi-disciplinary team.
- Solid knowledge of Southcoast communities, community resources and effective teaching/coaching skills.
- Knowledge of common chronic disease states and effective self management skills.
- Verifiable excellent driving record and reliable transportation.
- Community Health Worker certificate and have two job-related certifications including but not limited to: SHINE/CAC or SHIP certified, Peer Recovery Coach, Alcohol and Drug, Health/Life Coach, Mental Health Counselling, Behavioral Health, Integrative Behavioral Health, Licensed Addiction Counsellor, National Certified Addiction Counsellor Level I and II Certification.
- Agree to maintain all certification requirements while in the position.
- Massachusetts Vehicle Operators Class B license is required. A clear DMV driving record is also required.
Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
Compensation: Pay rate will be determined based on level of experience.
