What you will be doing:

• Provide direct patient care in accordance with the plan of care developed by the RN and physician.
• Administer medications, injections, wound care, and treatments as prescribed.
• Monitor and report vital signs, symptoms, and patient progress.
• Assist with personal care needs as appropriate to the nursing scope of practice.
• Maintain accurate, timely, and complete documentation in compliance with state and agency requirements.
• Promote safety and infection control for clients and coworkers.
• Recognize and report changes in client condition, as appropriate.

What we are looking for:


Requirements

• Current license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state(s) of employment.
• Graduate of an accredited school of professional nursing.
• Current CPR certification.
• Ability to work independently and manage time effectively.
• Strong verbal and interpersonal communication, observation, and critical-thinking skills.
• Compliance with HIPPA and all privacy and confidentiality policies.

Preferred
1 year of nursing experience in home health.

Additional Requirements
Valid California drivers license, auto insurance, and reliable transportation.
