Companion Care Needed

About who needs care: Had a stroke and heart attack about 1. 5 years ago. Wants to be independent. Has his own car that he cant drive it. An ideal day is one where he gets up, takes ashore walk to get his food, and possibly takes a shower. I am trying to get him to take a shower every other day. He can do it independently. About the care needs: He has a lot of medication and doctor appointments that he refuses to go to or take. I feel these are his choices to make but still want to try and get him to take them. Services needed include: meal preparation, errands / shopping, light housekeeping, feeding, and bathing / dressing.
