- Skills Checklist must be specific to the discipline listed on the job (interventional vascular/IR lab) – [required] \n
- Professional Reference/Reference Check - charge/supervisor/nurse manager/director/CNO/COO performance evaluation/reference from within 24 months of today's date [preferred] \n
- Driver’s License [document] [required] \n
- AHA/Active American Heart Association or ARC/AMERICAN RED CROSS BLS – [preferred] \n
- RCIS/CCI or ARRT/The American Registry of Radiology Technologists - [document] [required] \n
- Iowa Radiology license [document] [highly preferred] -- issuance timeline varies if non-active \n
