Occupational Therapist - Outpatient - (OT)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $1716.00 - $1785.00

Unit Notes Hospital Setting-Monday - Friday, rotating weekends Outpatient and inpatient roles, lymphedema eval experience a benefit. Would cover outpatient evaluations and split treats with COTAs, then assist on inpatient side for evaluations

Required Qualifications
Occupational Therapist, Acute
Experience: 1 year
Licenses: OT-KS(Copy needed)
Certifications: OTR-National Board for Certification in Occupational Therapy(Copy needed); BLS-AHA(Copy needed)
AND
Occupational Therapist, Outpatient
SSN Required
DOB Required
Work History: Should not be more than 30 days of gap in entire work history
References: 1 Reference in the last 12 months
