Allied Health Physical Therapist

Job Description:

Casa Real, a Skilled Nursing Facility in Santa Fe, New Mexico, is seeking a licensed Physical Therapist (PT) for a 13-week travel contract. This is a full-time role within a post-hospital, short-term rehabilitation and long-term care SNF setting. The PT will provide high-quality therapy services, working with patients recovering from illness, surgery, injury, as well as those needing ongoing rehabilitation in long-term skilled care.

Responsibilities:

• Conduct comprehensive patient evaluations and develop individualized treatment plans

• Deliver physical therapy services that include therapeutic exercise, functional training, gait and mobility work, manual therapy, and modalities as needed

• Assist with transfers, ambulation training, strengthening, balance, pain management, and other restorative interventions

• Educate patients, families, and caregivers in home exercise programs and mobility safety

• Collaborate daily with interdisciplinary SNF team including PTAs, OTs, SLPs, nursing staff, physicians, and other rehab staff

• Document patient progress, update plans of care, and ensure regulatory compliance in clinical documentation

Requirements:

• Active Physical Therapist license in New Mexico (or compact-eligible)

• Graduate of an accredited Physical Therapy program (DPT or equivalent)

• Minimum of 1 year of recent SNF, short-term rehab, or skilled nursing physical therapy experience preferred

• Current CPR/BLS certification (or as required by the facility)

• Strong clinical judgment, communication skills, and ability to manage caseloads independently in the SNF setting

Contract Details:

• 13-week travel assignment at Casa Real Skilled Nursing Facility, Santa Fe, NM

• Gross pay range: $2,000 – $2,240/week
