What You'll Do
As the LPN Intake Coordinator, you'll be the heartbeat of our office operations and intake process. Responsibilities include:

Answering phones and managing fax communications

Entering patient referrals into the EMR system

Assisting with coordination of home health services

Creating, modifying, and maintaining forms and records
-
Filing, distributing Medicare paperwork, and tracking physician orders

Assembling admission packets
-
Supporting quality assurance and regulatory compliance efforts

Jumping in to help your team whenever needed
Qualifications

Active LPN license (required)
-
Valid driver's license & reliable transportation

1+ year of home health experience (required)

Experience scheduling home health referrals (preferred)

Proficiency with Microsoft Office Suite (Word, Excel, etc.)

Ability to participate in on-call rotation
