LPN - Licensed Practical Nurse

Location: Mantey Heights Rehabilitation & Care Center – Grand Junction, CO
Schedule: Full-Time & Part-Time | 12-Hour Shifts (6:00 AM – 6:30 PM) & Evening Shifts (6:00 PM – 10:00 PM)

Position Overview

As an LPN, you will provide skilled nursing care under the direction of a Registered Nurse (RN) or physician. You will play a vital role in monitoring resident health, administering medications, and providing compassionate support to residents and their families.

Key Responsibilities

• Administer medications and treatments according to physician orders and facility policy.

• Monitor residents’ health, including vital signs and changes in condition, and promptly report concerns to the RN or physician.

• Assist with resident admissions, transfers, and discharges.

• Ensure accurate and timely documentation in medical records.

• Support CNAs and other team members by providing guidance and assistance.

• Promote and protect resident rights, dignity, and safety at all times.

• Collaborate with interdisciplinary team members to provide holistic resident care.

Qualifications

• Current LPN license in the State of Colorado (required).

• Ability to work 12-hour day shifts (6:00 AM – 6:30 PM) and/or short evening shifts (6:00 PM – 10:00 PM).

• Strong clinical and assessment skills.

• Excellent communication and teamwork abilities.

• A positive, compassionate attitude toward residents and coworkers.

• Previous experience in long-term care or skilled nursing preferred, but not required.
