Clinical Pharmacist Practitioner - Full-time

Summary: The Ambulatory Care Pharmacist practices within physician offices within Scotlands Health Care System to provide protocol-driven, evidence-based patient care. Job Duties: + Educates and consults with patients and providers regarding the use of medications, disease management, potential drug interactions, and monitoring of drug therapies. + Provides team-based care, including working with multidisciplinary teams, and care management may be involved. + Responds to requests for drug information from healthcare providers and patients. + Evaluates the usefulness of biomedical literature gathered and formulates responses to drug information requests based on analysis of the literature. + Provides medication-use education and counsels patients and caregivers. + Performs co-visits with patients and prescribes medications as appropriate. Qualifications: Minimum North Carolina Pharmacy License
