Mo Cara Hospice Care is dedicated to providing compassionate, patient-centered care to individuals at the end of their life journey. We offer comfort, dignity, and peace to both residents and their families. We are expanding our team and looking for a compassionate, skilled Hospice RN to join our team of caregivers committed to delivering exceptional hospice services.

Job Responsibilities:

• Administer medication and treatments to hospice patients in accordance with care plans.

• Conduct comprehensive nursing assessments for hospice patients.

• Provide emotional and physical support to patients and their families.

• Oversee and manage the health and comfort needs of patients in coordination with the hospice care team.

• Maintain accurate and timely documentation of patient care and progress.

• Support the development and implementation of care plans to enhance the quality of life for hospice patients.

• Respond and act appropriately in emergency or crisis situations.

• Provide education and support to family members regarding end-of-life care.

• Other duties as needed.

Job Skills & Qualifications:

Required:

• Currently licensed as a Registered Nurse (RN) in the State of Illinois.

Preferred:

• Previous experience in hospice care or palliative care.

• Experience working with individuals who are terminally ill, and their families.

• Ability to provide empathetic and professional care in end-of-life situations.
