Description
Purpose:
Medical Assistants collaborate within an Integrated Team Based Care (ITBC) model in the assessment and monitoring of participants holistic needs. Medical Assistants attend and facilitate educational in-service as needed. The Medical Assistant assesses and oversees medication monitoring and administration in compliance with applicable OAR’s and functions as a liaison between participants, medical providers, hospital staff, and other healthcare providers.
Accountabilities: (Responsibilities of the job)

Distributes medication in accordance with physician’s orders, applicable OARs, and New Narrative Policy
-
Administers injections on rotating basis per LMP orders
-
Monitors and reviews M.D.’s medication orders on a weekly basis and monitors the administration of medication to scheduled participants by program staff to ensure that dispensation is appropriate and in compliance with applicable OARs
-
Coordinates participant refill orders as needed

Tracks and documents participant height, weight, and vital signs
-
Completes and documents Patient Health Questionnaire during individual appointments with clients
-
Completes blood draws and UA’s as needed at the discretion of LMP or ITBC team
-
Follows up on participant LAB work, reviews LAB results, and ensures results are accurately reflected in the EMR
-
Coordinates as needed between LMP and participants PCP
-
Educates participants on medication names, dosages, functions, side effects, and self-administration
-
Completes billing notes for all billable charges consistent with Medicaid guidelines and within 72 hours of service completion

Follows guidelines in the medication policy. Constantly alert to changes in medication.

Follows up on identified health concerns of individual participants or reported concerns regarding a participant from the assigned LMP
-
Engages in de-escalation techniques
-
Communicates regularly with ITBC team members regarding participants health status
-
Assists in managing medical and pharmaceutical supplies
-
Conducts rehab activities around nutrition, including education on special dietary needs, exercise, weight management, healthy food choices, and portion size
-
Provides health education to participants in a participant centered, trauma informed manner, including use of harm reduction
-
Support in medication management and medical equipment trainings

Attend and positively participate in required meetings, including ITBC meetings and care coordination meetings with LMPS to support collaboration and efficiency

Adhere to any applicable ethics codes and New Narrative’s mission and values

Adhere to regular work schedule as set by direct supervisor

Follow through on all directives and instructions

Maintain current Oregon Driver’s License

Maintain CPR and First Aid Certificate

Education/Certificates/Licenses/Registration:
Medical Assistant degree required. Completion of Medical Assistant certification within six months of hire date.
Experience/Qualifications:

Experience working with individuals diagnosed with severe and persistent mental illness preferred
