POSITION TITLE: Team Assistant DATE: 10/07/2022
REPORTS TO: Hospice House Director
FLSA STATUS: Non-Exempt
SCHEDULE: Full Time
Essential Functions
Responsible for the initial and ongoing customer service experience for patients and visitors. Clerical duties, such as responding to telephone calls, communicating with patients and families through call system, greeting visitors and addressing their questions, and providing clerical assistance for supervisor and clinical staff.

Challenges in Position
8 hour shifts with 30 minutes for lunch on site. Must be skilled at prioritizing tasks. Ability to multi-task.
Frequent interruptions. May deal with emotionally distraught families. Interactions with terminally ill patients.
Requirement to maintain professional appearance and attitude at all times. No personal cell phone use at the desk.

Education and Experience
High school diploma or general equivalency diploma (GED) required. Minimum of two years of clerical experience in a healthcare environment preferred. Proficient in medical terminology, Microsoft programs (Word, Excel, Outlook), and general typing. Experience with an electronic medical record system is preferred.
Certificates, Licenses, Registrations, Vaccines
Valid COVID-19 vaccine required
Influenza vaccine during influenza season required
Valid drivers’ license, auto insurance, and good driving record is required if any driving is performed for work.
CPR Certification is required within 90 days of start of job.
Mathematical Skills
Basic ability to add and subtract.
Reasoning Ability
Ability to apply common sense understanding to carry out instructions furnished in written or oral form.
Language Skills
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to respond to questions from clients, customers, and the general public. Ability to write simple correspondence. Ability to speak professionally to patients, physicians and others in the organization via telephone and personal communication.

Position Requirements (not all inclusive)
Greet patients, families, visitors, staff, volunteers, and others with pleasant, helpful attitude.
Must be on duty at desk at assigned time, and leave promptly at assigned time, with minimal time away from the desk during the shift.
Type professional memos, letters and other information.
Maintain accurate daily census and staff assignment sheets and send electronically as required.
Maintain supply of forms utilized by hospice house staff.
Assist the charge nurse with distribution of selected data, accurately and appropriately, to include IDTM forms, plans of care, death notices, discharge forms and other documentation as needed.
Assist the hospice house staff with faxing, phone calls, other clerical duties as needed.
Utilize appropriate communication skills with co-workers and with other hospice departments.
Demonstrate a knowledge and understanding of the flow of information/communication from the hospice house to the administrative departments and impact on home and nursing home care staff.
Answer the telephone and call lights in a timely, professional manner and relay messages to appropriate staff, verbally and/or written, as appropriate.
Arrange transportation for patients going home from the hospice house; verify that respite return trips have been scheduled.
Obtain medical records from ER, hospitalizations, medical appointments in a timely manner, send to medical records for scanning.
Obtain procedure results (lab, x-ray) in a timely manner, send to medical records for scanning.
Maintain temperature logs on all HH refrigerators.
Distribute 90 day and annual employee evaluation packets to employee and unit director 30 days before evaluation due.
Participate in staff development (15 hours of CEUs of which 12 must be Hospice related) during each evaluation year as per Columbus Hospice policy.
Must attend at least one Memorial Service during each evaluation year as per policy.
Complete assigned duties in a timely, proficient manner.
Attend staff meetings or other educational in-services, as required per policy.
Adhere to professional hospice dress code.
Ability to perform multiple tasks at one time.
Maintain an organized, clean, HIPAA-compliant workspace.
Take all reasonable precautions to maintain privacy and non-disclosure of any confidential information about patients, other staff members, and the general operations of Columbus Hospice.
Represent the company in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers.
Complete all job responsibilities in a thorough and complete manner, utilizing good judgment in all aspects of the job.
When unsure of a directed task or any part of job responsibilities, proactively seek the assistance of immediate supervisor.
Ensure job responsibilities are met, including attendance requirements and prompt arrival at work.
Assist with special projects, as requested.
