Job Title: Physical Therapist
Full-time, flexible Monday-Friday schedule.
New grads welcome- we provide excellent training and mentorship to set you up for success!
Starting wage: $34-55/hr. depending on experience.
Up to $50/hr. for admitting new clients.
Travel stipend available.
Location: 915 Centre Avenue Suite 2 Fort Collins, Colorado

About the Role: As a Physical Therapist at Columbine Poudre Home Care, you will:
Assess, plan, and provide skilled PT services tailored to each patient’s goals and environment.
Restore mobility, manage pain, and improve overall function and safety at patients’ homes.
Communicate effectively with medical staff, clients, and families regarding treatment goals and plans to help maintain patient independence.
Participate in staff meetings, discharge planning, and provide in-services as needed.
Refer clients to outpatient services and discontinue therapy when appropriate.
Perform physical therapy services for other agencies as contracted by CPHC.
What We're Looking For:
Experience:
Graduate of accredited school of Physical Therapy, Bachelor’s or greater degree required.
Licensed by the State of Colorado as a Physical Therapist.
1+ year of experience as a Physical Therapist.
Skills:
Patient assessment and diagnostic skills.
Manual therapy and therapeutic techniques.
Communication skills.
Documentation and record-keeping.
Technology and equipment proficiency.

Our Total Rewards package includes a salary range of between $34 - $55/hr. DOE
