Description
Responsible as a team leader for the development and implementation of individualized programs and services for individuals served in Day Habilitation. Service areas emphasizing community integration and in-house activities to enhance the person's chosen goals affecting his/her capacity to live, work, socialize, and participate within the community.
Essential Duties and Responsibilities
1. Directs services as a team leader to people served based upon their choice and person centered planning in an individualized service plan.
2. Responsible for assessments, development, and writing of the Day Habilitation plans and addendums as needed. Also responsible for attendance records, transportation data, daily documentation, monthly summaries, and prepare for and attend annual and semi-annual reviews of the Individualized Service Plans (ISP). Responsible for all mandated/required documentation in the individuals case records.
3. As a team leader, directing staff to coordinate and implement all daily activities including community integration, in-house activities, skills training, guest speakers, volunteer activities, adaptive technology, and physical assistance.
4. Serves as team leader directing and assisting, as needed, in providing services to people for their physical and personal hygiene needs. Follows diet and eating/feeding protocols, repositioning, or ambulation as prescribed by a physician or PT consult. Proven competency with lifting using a variety of mechanical lifts.
5. Responsible to report information regarding people's well being to the designated supervisor re: advocacy, health, and emotional issues. Communicate any or all issues to the appropriate professional.
6. Adherence to all rules, regulations, policies and procedures of Federal, State, and Agency Programs.
7. Assists with a.m. and p.m. bussing of participants entering and exiting building. Possible daily driving of people served in Agency vehicles. This also includes the proper tie downs of wheelchairs and operation of lifts for loading and unloading.
8. Responsible for maintaining van logbooks, program equipment, Day Habilitation rooms, and vehicle cleanliness and equipment. May include washing windows, removing snow and steam cleaning interiors.
9. Supports and enhances the People served as the major decision makers affecting their services. Support Self-Advocacy and advisory committees.
10. Participate in curriculum development activities committee

Valid and Clean New York State Driver's License required
High school diploma or general education degree (GED); two years of experience in related field working directly with persons with Developmental Disabilities preferred.
