Supported Living Case Manager

Work Inc. is seeking a full - time Supported Living Case Manager to provide meaningful, person-centered support to adults with Developmental Disabilities and Mental Illness. As a Case Manager, you’ll work directly with individuals in their own apartments—helping them build skills, access resources, and create fulfilling lives. Every day is unique, rewarding, and filled with opportunities to make a lasting impact.
Duties and Responsibilities
- Provide comprehensive Case Management services in alignment with each individual’s Individual Support Plan (ISP).
- Provide an equitable service delivery system that respects and responds to the diverse needs of individuals
- Support individuals in achieving full integration into the community.
- Assisting individuals with accessing and applying for benefits/entitlements
- Facilitate access to ancillary support services
- Providing information, referral, assisting individuals to navigate local resources
- Support individuals in maintaining optimal health by managing medical appointments,
- Assists individuals in money management support
- Nurturing and maintaining positive relationships a
- Assists individuals in maintaining a safe, clean, and comfortable living environment.
- Provides emergency support outside of standard operating hours, as required.
Skills and Abilities
- Have proficient computer skills and knowledge of Microsoft 365
- Ability to pass First Aid/CPR Training
- Must have good written and verbal communication skills.
- Good interpersonal skills with polite and courteous demeanor in all interactions.
- Ability to effectively communicate with families/guardians and funding agencies and ancillary services.
Qualifications
- Bachelor’s degree in education, Special Education, Social Work, or related field/or five-plus years of related experience with a High School Diploma or equivalent.
- Possession of a valid Massachusetts Driver’s License. Safe driving record and use own vehicle to transport individuals.
- Acceptable CORI/ National Criminal Background Check (NCBC), DPPC Database, and Driving Record.
