Licensed Practical Nurse - Full-time

Job Summary:

JCCA is seeking a qualified Licensed Practical Nurses to provide primary health care management for assigned cases collaborating with all members of health care and clinical team.

Summary of Job Functions:

• Provide primary health care management for assigned cases.
• Maintain accurate and up-to-date Medication Administration Records
• Actively communicate with community providers scheduling needed appointments and obtaining health related information for clients
• Communicate and coordinate with clinical team, foster and biological family/guardian about all medical conditions, appointments and/or treatments.
• Provide formal and informal training for foster and  biological parents on specific health care needs of youth, including appropriate health practices and administration of medications.

Qualifications:

• NYS Licensed Practice Nurse required          
• Pediatric and/or Medical/Surgical  experience
• Excellent organizational skills and follow up.
• Effective communication skills - both written and oral
