Job Details

We’re seeking a Hospice Team Manager (RN) to lead a dedicated interdisciplinary hospice care team. This hybrid leadership role involves clinical oversight, team supervision, and coordination of patient-centered care plans.

Key Responsibilities:

Provide clinical and operational leadership for the hospice team

Oversee patient care plans and ensure adherence to regulatory standards

Serve as a liaison among team members, patients, families, and physicians

Mentor and support nursing staff to promote quality outcomes and team collaboration

Participate in on-site visits (2 days/week) and work remotely the remaining 3 days
