Job Summary
Folkston Park Care & Rehab is in search of a dedicated and compassionate Physical Therapist Assistant (PTA) to join our IN-HOUSE team in a long-term care (LTC) facility! Under the supervision of a licensed Physical Therapist, the PTA will assist in providing physical therapy services to residents in order to enhance their quality of life, restore mobility, and promote functional independence.

Key Responsibilities:

  • Implement physical therapy treatment plans developed by the Physical Therapist in accordance with physician orders and facility protocols.
  • Provide therapeutic interventions such as exercises, gait training, balance activities, and use of assistive devices.
  • Monitor patient progress and report any changes or concerns to the supervising Physical Therapist.
  • Assist residents with mobility and physical functioning to support daily living activities.
  • Educate residents, caregivers, and staff on mobility techniques and use of adaptive equipment.
  • Maintain accurate and timely documentation of treatments, progress notes, and other required records.
  • Ensure a safe, clean, and supportive environment for all residents.
  • Collaborate with interdisciplinary team members including nurses, occupational therapists, and speech-language pathologists to support resident care plans.
  • Adhere to all federal, state, and facility regulations and policies.

Qualifications:

  • Associate degree from an accredited Physical Therapist Assistant program.
  • Current state licensure or certification as a Physical Therapist Assistant.
  • Prior experience in a long-term care, skilled nursing, or geriatric setting preferred.
  • Strong communication, organization, and interpersonal skills.
  • Ability to work effectively as part of a team and independently with minimal supervision.

Working Conditions:

  • Primarily in a skilled nursing or long-term care facility setting.
  • Physical demands include lifting, standing, bending, and assisting with patient mobility.
