Position Title: Physical Therapist
Location: Woodrow Wilson Keeble Memorial Health Care Center (WWKMHCC), 100 Lake Traverse Drive, Sisseton, South Dakota 57262
Schedule: Monday–Friday, 8:00 AM – 4:30 PM (no weekends/holidays); 40 hrs/weekKey Responsibilities

• Provide evaluation and treatment for patients with diverse conditions (neurological, musculoskeletal, post-surgical, prenatal/postpartum, chronic pain, etc.).

• Develop individualized treatment plans, set goals, and involve patients and families in care planning.

• Deliver physical therapy interventions and adjust treatment regimens through periodic evaluations.

• Collaborate with referring physicians and participate in the Patient Centered Medical Home (PCMH) model of care.

Qualifications:

• Graduate of an accredited Physical Therapy program.

• Current, unrestricted license to practice Physical Therapy in at least one U.S. state or territory.

• Minimum of 1 year of recent experience (within the past 36 months) as a Physical Therapist.

• Current Basic Life Support (BLS) certification; ACLS preferred.

• Experience with Electronic Health Records (EHR).
