Position Title: Social Service Provider 2, Non-Licensed - Behavioral Health and Developmental Disabilities Support Specialist (HCP122)

List positions supervised through immediate contact: 3 FTE BH Supported Employment Specialists

  1. QUALIFICATIONS:
  • Education:
    • Entry qualifications include a bachelor's degree in the social sciences/helping professions and at least one year experience delivering supported employment services

OR certification by a nationally or state-recognized evidence-based SE training program.

  • Experience:
    • Experience in providing case management, vocational assessments, job development, job coaching, and ongoing support to assist individuals to achieve personal goals and enhance quality of life.
    • Possesses a strong understanding of supported employment principles and practices, knowledge of community resources, and job development strategies.
    • Works independently and part of a team, working directly with individuals, families, and interdisciplinary teams to create personalized plans, ensure access to necessary services, and promote competitive integrated employment and community integration.
    • At least 1 year of experience providing employment services.
  • Skills and Competencies:
    • Provides comprehensive support to individuals with intellectual and developmental disabilities (IDD), behavioral health (BH) concerns, and those seeking employment.
    • Performs needs assessments of program services and assesses the needs of individuals in services.
    • Possess knowledge of agency and federal policies, procedures, and guidelines.
    • Oversees compliance with all regulations of assigned programs.
    • Ability to establish and prioritize goals and objectives for assigned programs and identify needs and risks.
    • Ability to work with a diverse population with various modes of intervention and techniques.
    • Must possess a valid driver's license and reliable transportation to travel to work sites.
    • (Physical) No lifting of more than 20 pounds.
  • Certifications (if applicable):
    • ACRE Certification preferred

F. PERFORMANCE IMPROVEMENT:

G. ASSIGNED DUTIES:

  • Provide Oversight and Supervision for IDD and BH Supported Employment Services
    • Supervise staff in both specialty areas
    • Assure team members have appropriate credentials and training
    • Operate programming in line with fidelity to the models for both IDD and BH Supported Employment and for GVRA.
  • Employment Treatment Plan Development
    • Conduct comprehensive assessments to evaluate the needs, strengths, and preferences of individuals with IDD and BH concerns.
    • Develop, implement, and review individualized employment plans and goals based on the client's needs, preferences, and progress.
    • Ensure employment plans are person-centered, culturally appropriate, and align with evidence-based practices.
  • Job Development and Search Support:
    • Identify and engage potential employers, develop job search plans, and match individuals with appropriate job opportunities aligned with their abilities and goals.
    • Assist individuals with resume building, interview preparation, and identifying suitable job openings.
    • Provide on-the-job coaching and support to ensure individuals learn job-specific skills and meet employer expectations.

Position Number:

  • Ongoing Support and Job Retention:
    • Deliver continuous support to individuals, teaching self-management, communication skills, interpersonal skills, and maintaining job performance.
    • Assist individuals in retaining employment and transitioning to new jobs if desired, including redefining vocational goals and providing reemployment support.
    • Support individuals interested in self-employment, including business planning and identifying necessary supports.
  • Oversight for Case Management and Coordination:
    • Act as the primary point of contact for clients and families, providing advocacy and ongoing support.
    • Assure coordination of services with other professionals, including mental health treatment team, community partners and GVRA, to ensure holistic client care.
    • Monitor client progress and adjust employment plans as necessary.
  • Documentation and Compliance:
    • Maintain accurate and timely documentation of client interactions, treatment plans, progress, and other required reports.
    • Ensure compliance with all relevant local, state, and federal regulations, including HIPAA and confidentiality requirements.
    • Complete documentation, ensuring proper reporting of services.

H. JOB RESPONSIBILITIES:

  • 60% Dedicated to BH Supported Employment Operations
  • 40% Dedicated to IDD Supported Employment Operations
