Registered Nurse Case Manager

Location: Martinsville, VA
Schedule:
Option 1: Monday–Friday, 8-hour days
Option 2: Four 10-hour days (e.g., Mon–Fri, 8 AM–6 PM or 7 AM - 5 PM)
Weekend Rotation: Every 8th weekend

Mountain Valley Hospice & Palliative Care is seeking a compassionate and skilled Registered Nurse Case Manager to provide personalized care to patients and families in Forsyth and Davidson Counties. As part of our interdisciplinary team, you will deliver hands-on nursing care, coordinate services, and support patients wherever they call home—whether in a private residence, assisted living, or skilled nursing facility.

Key Responsibilities

• Provide direct nursing and supportive care for terminally ill patients.

• Assess, plan, and manage individualized care in collaboration with physicians, social workers, chaplains, and CNAs.

• Recognize and respond to changes in patient condition, ensuring timely communication and interventions.

• Educate and guide patients, families, and caregivers with compassion and clarity.

• Support and mentor CNAs in delivering high-quality, patient-centered care.

Qualifications

• Current RN license in NC (multistate preferred).

• Hospice or home health experience preferred.

• Strong clinical assessment skills with the ability to work both independently and within a team.

• Compassionate communication and time management skills.
