PRN Physical Therapist

Legacy Living Care is seeking a licensed Physical Therapist (PT) to provide home health therapy services to patients in the Macon, GA
This is a PRN / per-visit opportunity ideal for therapists seeking flexible supplemental work.

Responsibilities

• Perform initial physical therapy evaluations
• Develop and implement individualized treatment plans
• Provide therapeutic exercises, gait training, and mobility improvement
• Educate patients and caregivers on fall prevention and home safety
• Complete timely documentation

Requirements

• Active Georgia Physical Therapist license
• Home health experience preferred but not required
• Ability to travel to patient homes in Macon, GA
• Reliable transportation
• Strong clinical and communication skills
• Accepting Chattanooga TN area as well
