Job Description
We're looking for a dedicated Community Health Worker (CHW) to join our Integrated Care Team supporting patients in the Lawrence and Haverhill area. This position plays a vital role in connecting patients to the care, resources, and support they need to improve their overall health and well-being.
As a CHW, you'll conduct screenings for health-related social needs, help patients navigate complex systems, and create individualized care plans that address barriers such as housing instability, food insecurity, transportation challenges, and access to community programs. You'll engage directly with patients in their homes and communities, often building long-term, trusting relationships that empower them to achieve their personal health goals.
This is an excellent opportunity for someone who enjoys helping others and thrives in a role that blends healthcare, case management, and community advocacy. You'll collaborate closely with an Integrated Care Team of nurses, social workers, and primary care providers - ensuring every patient receives coordinated and compassionate care.

Qualifications:
- High School Diploma or equivalent
- Bilingual in Spanish and English
- Background in community support, care coordination, or as a Medical Assistant preferred
- Valid driver's license and reliable transportation for community visits
- Compassionate, self-starter with strong communication and problem-solving skills
