Responsibilities

Manage the provisions of on-site supportive services for assigned properties. Connect residents to the supportive services needed in order to continue living safely in an independent setting.

Duties and Responsibilities

1. Provide direction and resources to residents to enhance their independence and safety. Assist residents with determining insurance costs, balancing checkbooks and hiring care providers

2. Assess residents' ability to safely maintain independence and recommend services as needed

3. Provide information to residents and their designee regarding available resources and access procedures

4. Assist elderly or disabled community clients with application requirements, linking them with services to remain independent and advocate on their behalf

5. Coordinate and implement wellness education and enrichment programs to educate and support healthy lifestyles of residents

6. Establish and maintain resident files, maintain contact notes, implement service management plan and other required documentation to monitor progress and comply with program requirements

7. Collect and establish a community resource directory of providers for use by staff and residents

8. Assist with implementation of volunteer support programs

9. Collaborate with management in resolving resident disputes


Qualifications

Job Qualifications

Education, Training, and Licensure/Certification

  • Bachelors Degree in Social Work, Gerontology, Psychology or Counseling OR five years' experience in social service delivery with senior citizens and non-elderly disabled

Experience

  • Three years' experience in social service delivery with senior citizens and non-elderly disabled preferred

Knowledge Skills and Abilities

  • Knowledge of Housing and Urban Development (HUD) administrative policies and current industry developments
  • Knowledge or training related to the aging process, elderly or disability services with ability to apply principles to solve practical problems and deal with variables
  • Ability to communicate effectively orally and in writing; prepares business letters, summaries and reports using prescribed format and conforming to all rules of punctuation, grammar, and diction
  • Ability to perform basic arithmetic calculations
  • Ability to work with and demonstrate compassion with vulnerable elderly persons
  • Ability to communicate effectively and collaboratively to representatives of HUD and other federal, state and local community assistance organizations
  • Familiarity with various local community resources
  • Demonstrated ability to advocate, organize, problem-solve and provide results for residents served
  • Ability to effectively represent program and center through public speaking to community and referral agencies/groups
  • Ability to effectively utilize word processing software and American Association of Service Coordinators (AASC) Online Software

Additional Work Requirements

  • Ability to work a flexible work schedule
  • Statewide travel for training is required
