What You’ll Do

• Translate scientific language into clear, understandable information to help patients take charge of their health
• Assess patient needs and readiness to adopt healthier behaviors
• Develop, implement, and improve educational materials and tools that inspire positive change
• Conduct health education sessions with patients and families, and participate in community outreach
• Assist in training and mentoring Community Health Workers
• Collaborate closely with care teams to support patient health goals
• Lead tobacco cessation programs and travel between service locations as needed

Qualifications

• Bachelor’s degree in public health, Health Education, Nursing, or related field
• Minimum 2 years’ experience in community health education, outreach, or similar roles
• Valid driver’s license and current CPR certification
• Excellent interpersonal and communication skills, with leadership and independent learning abilities
