Job Title: Prevention Services Coordinator - Knoxville
Reports to: Prevention and Outreach Manager
Status: Full Time
Compensation: $21/hour
Position Summary:
Positively Living and Choice Health Network is hiring a Prevention Services Coordinator to assist the Prevention and Outreach Manager with PrEP navigation and case management, HIV and HCV testing, outreach events, and state reporting. This position will work closely with other staff across the organization (including clinical, case management, and harm reduction team members) to coordinate care for prevention clients. The Prevention Services Coordinator’s efforts will focus on creating rapport with clients as well as providing education on safer sex, HIV and STI prevention, and risk reduction. This position should be adaptable, team-oriented, knowledgeable, and passionate about the work.
Responsibilities and Duties:
· Develop rapport, establish trust, and maintain confidentiality to create a safe, nonjudgmental space for clients.
· Ensure understanding of clinic flow, appointments, laboratory testing, and medication refills.
· Conduct and implement HEAT training with Prevention and Outreach Manager.
· Manage walk-in HIV and Hep-C testing and PrEP intakes.
· Provide HIV and Hep-C testing alongside Choice Health Network’s Harm Reduction Team
· Assist Prevention and Outreach manager with outreach and events.
· Perform data entry for HIV and HCV testing as well as within state and grant reporting portals.
· Troubleshoot pharmacy issues (contacting pharmacies, continuum of care management, and communications with clients)
· Utilize EMR system to manage caseload and communicate internally, document patient communications, contribute to agency’s 340B coordination efforts, provide support for monthly, quarterly, and annual reports on prevention/harm reduction services and outreach, assist with quarterly satisfaction survey data collection and analysis.
· Liaison between prevention, clinic staff, harm reduction, and occasionally case management.
· Provide ongoing support to clients in PrEP caseload (by phone, email, and in person) to ensure medication adherence, continuous enrollment in financial assistance programs, and insurance issues.
· Be able to communicate assertively and effectively with clients and staff alike.
Job Qualifications and Skills:
· The ability to effectively communicate as part of a team, as well as work independently.
· Comfortable providing healthcare services through a social justice framework.
· Computer and technology literate (comfortable with regular Word and Excel use).
· Ability to concisely document client interactions within an electronic medical record system.
· Ability to follow HIPAA rules and regulations to respect the privacy of all clients.
· The ability to set firm boundaries and practice assertive communication.
· The ability to put your own personal judgments, prejudice, and biases aside and work with clients with diverse life experiences.
Education, Experience, and Requirements:
· Bachelor’s Degree or two years of experience in a related field.
