About the Role:
The Intake Specialist plays a crucial role in the business by serving as the first point of contact for clients seeking services. This position is responsible for gathering essential information from clients to assess their needs and determine appropriate services. The Intake Specialist ensures that all necessary documentation is completed accurately and efficiently, facilitating a smooth transition into care. By effectively communicating with clients and healthcare providers, the Intake Specialist helps to streamline the intake process and improve overall client satisfaction. Ultimately, this role contributes to the organization's mission of providing high-quality care and support to those in need.
Minimum Qualifications:
- High school diploma or equivalent; associate's or bachelor's degree in a related field preferred.
- Previous experience in a healthcare or social services setting, particularly in an intake or administrative role.
- Valid Drivers License, Fingerprint Clearance Card, TB Test, CPR and First Aid
- Must be able to stand, walk, or sit for extended periods of time and be able to lift 25-50 lbs
- Must be able to work in inclement outdoor weather
Preferred Qualifications:
- Certification in healthcare administration or a related field.
- Experience with electronic health records (EHR) systems and data entry.
Responsibilities:
- Conduct initial assessments and interviews with clients to gather relevant information regarding their needs and circumstances.
- Complete and maintain accurate records of client information, ensuring compliance with privacy regulations and organizational policies.
- Coordinate with healthcare providers and social service agencies to facilitate referrals and access to necessary services.
- Provide clients with information about available resources and support services, guiding them through the intake process.
- Monitor and follow up on client progress, ensuring that they receive the appropriate care and support throughout their journey.
- Identifies openings in schedule, notifies supervisor of gaps in schedule that decrease efficiency, actively works with marketing in order to notify partners of openings and availability.
- Prepares for sessions daily and assists in check in and check out of clients
- Performs other related duties as assigned or requested.
