This position serves as a vital liaison between patients, healthcare providers, and administrative teams to coordinate and facilitate patient services efficiently. The coordinator is responsible for managing patient intake, scheduling, and follow-up care, ensuring compliance with healthcare regulations and organizational policies. By providing compassionate support and clear communication, the role enhances patient satisfaction and promotes positive health outcomes. Ultimately, the LVN Patient Services Coordinator contributes to the seamless operation of home health services, supporting both clinical and administrative functions.

Minimum Qualifications:

  • Valid Licensed Vocational Nurse (LVN) license in the state of Texas.
  • Minimum of 2 years of experience in a nursing or patient care coordination role, preferably in home health or related healthcare settings.
  • Strong knowledge of medical terminology, patient care procedures, and healthcare regulations including HIPAA.
  • Proficiency in electronic health record (EHR) systems and standard office software.
  • Excellent communication and organizational skills.

Preferred Qualifications:

  • Experience working in home health care or community health services.
  • Familiarity with insurance authorization processes and billing procedures.
  • Certification in patient care coordination or case management.
  • Bilingual abilities, particularly in English and Spanish, to better serve the local community.
  • Demonstrated ability to work effectively in interdisciplinary healthcare teams.

Responsibilities:

  • Coordinate patient admissions, scheduling, and discharge planning in collaboration with clinical staff and patients.
  • Serve as the primary point of contact for patients and families, addressing inquiries and providing information about services.
  • Maintain accurate and up-to-date patient records in compliance with HIPAA and organizational standards.
  • Assist nursing staff with clinical documentation and ensure timely communication of patient care plans.
  • Collaborate with interdisciplinary teams to facilitate continuity of care and resolve any service-related issues.
  • Monitor patient progress and follow up on care plans to support positive health outcomes.
  • Ensure adherence to regulatory requirements and assist with quality assurance initiatives.
