Job Description
We are seeking a Licensed Clinical Social Worker (LCSW) to join our collaborative healthcare team and make a meaningful impact on the lives of residents in skilled nursing and rehabilitation facilities throughout Georgia. This is a unique opportunity to work alongside dedicated healthcare professionals in one of Georgia's most respected healthcare organizations.
Position Summary: As the LCSW, you will conduct initial evaluations, provide psychotherapy, and offer behavioral management to residents in long-term care and nursing facilities. You will collaborate with an assigned Psychologist and/or Psychiatrist/PMHNP at each facility to ensure continuity of care, with a focus on both psychological and psychopharmacological treatment.

Responsibilities:
- Perform evaluations and ongoing therapy sessions
- Provide evidence-based therapies such as CBT, ACT, and DBT
- Collaborate with prescribers to manage patient care
- Maintain comprehensive, timely documentation in EHR
- Build and maintain strong therapeutic alliances with a diverse patient population
- Travel to multiple facilities within your assigned territory
Required Qualifications:
- Current LCSW
- Active, unencumbered Georgia license
- Proficiency in evidence-based therapeutic techniques (e.g., CBT, ACT, DBT)
- Strong computer and technology skills
- Current CPR certification
- Valid Georgia driver's license and willingness to travel
- Ability to multitask, prioritize, and make independent decisions within a collaborative team environment
- Excellent communication and interpersonal skills
