Geriatrics Nurse Practitioner

Contract to provide home care for a Medicare/Medicaid plan in Fulton County, GA

• Conduct comprehensive histories and physical exams, evaluate social determinants, and deliver health education.
• Order diagnostics, develop and adjust treatment plans, and manage medications. (as applicable)
• Educate on disease prevention, medication use, lifestyle changes, and self-care
• Complete and submit visit documentation within 24 hours (this should be completed during the visit and shouldn't have much to do at home).

Looking for individuals that are open to in home work. Open to providers that can give a day or a bit of availability each week or someone that can be full time.

Open to nights and weekends as well.

Ideal candidates will have home health background
