🩺 Job Title: Nurse Practitioner – Assisted Living / Geriatric Care
Location: Klamath Falls, Oregon
Schedule: Monday–Friday, 8 AM–5 PM
Work Environment: Assisted Living, Post-Acute Rehab, Memory Care
Type: Full-time or Part-time (Flexible hours)
🧑⚕️ Key Responsibilities
- Provide primary care services to geriatric patients in assisted living and post-acute settings.
- Conduct patient assessments, develop care plans, and manage chronic and acute conditions.
- Collaborate with facility staff including nurses, therapists, dieticians, and social workers.
- Maintain accurate documentation using Epic EMR.
- Respond to emergent needs and assist in urgent clinical situations.
- Build long-term relationships with patients and families.
🎓 Qualifications
- Active, unrestricted Oregon NP license (or ability to obtain).
- Board certified or board eligible.
- Background in Family, Adult, or Geriatric care preferred.
- New graduates are welcome to apply.
💼 Benefits
- Salary: $110,000–$155,000/year (based on experience)
- No weekends or on-call requirements
- Relocation/sign-on bonus up to $5,000
- CME allowance: $2,000/year
