Position Summary:
We are seeking an experienced RN Case Manager with 2+ years of acute care experience, including Care Coordination and Discharge Planning in a fast-paced healthcare environment. The ideal candidate will collaborate with interdisciplinary teams to ensure efficient patient care, facilitate safe transitions of care, and coordinate appropriate post-discharge services. Strong communication, critical thinking, and organizational skills are essential for success in this role.
Roles & Responsibilities:
- Coordinate patient care plans and collaborate with physicians, nurses, and interdisciplinary teams to ensure quality outcomes.
- Perform comprehensive assessments and develop individualized discharge plans based on patient needs and available resources.
- Facilitate timely transitions of care, including referrals to rehabilitation facilities, home health services, and community resources.
- Monitor patient progress, evaluate barriers to discharge, and implement solutions to support efficient care delivery.
- Maintain accurate documentation, communicate with patients and families, and ensure compliance with regulatory and hospital requirements.

- Charting System: Epic Required
- Valid CA State RN license
- 2 Years of recent experience required as an RN working in a acute care setting.
