POSITION SUMMARY

To provide ongoing therapy to residents in a timely manner per physician’s orders. Communicate progress within the facility of resident progress. The Speech Language Pathologist evaluates and treats patients, communicates with families, physicians and other health team members and maintains appropriate documentation of services including MDS responsibilities complying with State and Federal regulations. Participates in on-going facility meetings and conducts in-service and training for facility staff on ongoing basis in restorative and maintenance techniques.

QUALIFICATIONS

  • Minimum of a Master’s Degree in Speech Pathology, with Certificate of Clinical Competence (CCC), or the equivalent of minimum of one year experience working with adults with disorders of neurological origin, including language comprehension, expression, cognition, articulation, voice, fluency, swallowing, and hearing. The candidate must also have the ability to function independently within the department.
  • Valid license in Speech Pathology in the state (where applicable) or actively accumulating the supervisory experience required for license
  • Registration with the Department of Health or eligible.

Preferred qualifications:

  • Able to utilize computers, Servox, augmentative devices and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
  • Member of Speech Language Hearing Association.
  • Auditorily and visually integrate and synthesize large amounts of information, (i.e., in-service, evaluations, and therapy tasks).

ESSENTIAL FUNCTIONS:

  • Evaluates and develops appropriate treatment plans to restore or improve communication efficiency for the residents within twenty-four (24) hours of physician referral.
  • Evaluates residents and formulates effective treatment plan per physician request and obtain approval for services from referring physician and third party payors.
  • Initiates ordered treatments and communicates with physician regarding resident’s progress.
  • Documents evaluations, weekly progress notes, discharge summaries, and other pertinent information as required by the facility’s policies, third party payers and stare and federal regulatory agencies.
  • Maintain proper documentation according to state, federal and third party payor regulations.
  • Maintain proper records according to facility policy and procedures.
  • Communicates and collaborates with team members regarding resident’s progress, problems and plans.
  • Participates in rehab conferences at regular intervals.
  • Participates in resident care planning conferences.
  • Provides resident, family, and staff with education as needed.
  • Performs other duties or special assignments as directed by the Director of Rehabilitation.
