Client Specific DSP-FDS

Nick, a 29-year-old male seeking a direct support professional to help him engage with the community. Services will include transportation, identifying activities of interest in the community, support to engage in these activities, and some personal care/household assistance as needed. His interests include music, playing the guitar, animals, and going to the park.

20 hours a week, $18/hr.

Mileage reimbursement up to 50 miles a week.

Requirements:

• Access to reliable transportation and adequate car insurance.
• A clean background check is required prior to employment.
• Prior experience working for individuals with developmental disabilities.
• Ability to work together with the client to complete organizational tasks.
• Ability to carry out individual service goals and communication goals.
• Assistance with transportation if needed to dr. apts. and errands.
• Household organization and cleaning
• Documentation of services
• Consistent contact with Case Manager in re: to program activities.
