Requirements **Required for presentation: must have actual state license in hand, Must be board certified, and must have TX DEA in hand**""Completed applications, updated CV, government photo ID, state medical license and DEA (TX address), DD214 (military), board certification , education and training verification (letter of completion and competency accepted if new resident or graduate), ECFMG (if applicable), case logs (24 months), CMEs (24 months) - 4 stroke education hours annually and 9 trauma for a total of 27 during the 3-year survey period, required life cards per service line, TX CPA, license link for NP/PA, 3 peer references, ACLS (if not Board Certified EM ), ATLS, NIHSS.. If immunization record is not available, a positive titer for each is required.. Hepatitis B (Series of 3 doses). Measles/Mumps/Rubella (Series of 2 doses). Varicella/Chickenpox (Series of 2 doses). Tdap (TETANUS, DIPHTHERIA ACELLULAR PERTUSSIS) within last 10 years.. Flu Vaccine for 2024-2025 season. Or let me know if you need a declination form. (Religious or Medical reasons). TB completed within the last year.. CSM will accept - (QuantiFERON, QuantiFERON-TB GOLD of T-Spot) or a 2-step skin test.. Chest X-Ray if positive and a questionnaire that CSM will provide. ""Board certified EM physicians preferred but facility will accept FM/IM trained w/ ED experience EMR: EPICPhysician Coverage: 7a-7p & 7p-7a daily
