Job Description
American Traveler is seeking a Physical Therapist with at least 1 year of recent home health experience and expert OASIS skills for a day-shift home health assignment.
Responsibilities
- Home health setting with 1:1 patient assignment
- Territory-based caseload, typically visiting 4-6 patients per day
- Standard productivity is 27-30 productivity points based on territory
- Day shift schedule, working 8-hour shifts from 8:00 to 16:30
- No weekend or on-call requirements
- Documentation tools provided and must be returned at the end of assignment
- Timely communication and documentation are mandatory
- The system efficiently groups clients to minimize driving and windshield time
- Maximum radius may occasionally reach up to 100 miles in territory
- Completion of the Hospice/Home Health Checklist is required
- All travelers must have RTO submitted at time of application
- First-time travelers are welcome to apply
- Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
- Travel pairs are accepted, but each traveler will be assigned their own territory and must have separate transportation
Requirements
- Minimum of 1 year recent skilled home health physical therapy experience within the past 3 years
- Current BLS certification required
- Must hold an active WA license or compact license
- Expertise in OASIS required
- Proficiency in HCHB (Homecare Homebase) is preferred
- Active driver's license and personal transportation required
- License required if applicable
