Ok to license• DEA: AZ registered• CSR: not required• Certifications Required: ACLS• Minimum Experience Required: 1 yearCredentialing Timeframe: 30 daysOT: 1.5 normal rateOT Billing Rule: after scheduled shiftHoliday: 1.5x normal rate - New Year s Day Memorial Day Independence Day Labor Day Thanksgiving Day Christmas Day 00 59 .
