About the Role:

As a Physical Therapist within the Health Care Services industry, you will play a critical role in helping patients recover from injuries, manage chronic conditions, and improve their overall physical function. Your primary objective will be to design and implement personalized rehabilitation programs that address each patient’s unique needs, focusing on restoring mobility, reducing pain, and preventing further injury. You will work closely with patients who have orthopedic conditions, sports-related injuries, or other physical impairments, utilizing evidence-based manual therapy techniques and therapeutic exercises. Collaboration with physicians, occupational therapists, and other healthcare professionals will be essential to ensure comprehensive care and optimal patient outcomes. Ultimately, your work will empower patients to regain independence and enhance their quality of life through targeted physical rehabilitation strategies.


As a Physical Therapist, you will:

  • Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.

  • Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.

  • Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.

  • Applies concepts of infection control/universal precautions in performing patient care

  • Assure continuity of quality patient care delivered with appropriate documentation

  • Provide effective communication to patient/family, team members and other health care professionals

  • Promote agency philosophy and administrative policies

  • Maintain and complete all documentation within company stated guidelines.


Requirements

  • Successful completion of accredited physical therapy program and state licensure exam.

  • Valid Physical Therapy license

  • 1 year of home health experience preferred

  • CPR certification

  • Valid Driver’s license

  • Valid Auto Insurance

  • HomeCare HomeBase experience preferred

Physical Demands

  • Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.

  • Must be able to lift, carry, pull, or push up to 100 lbs..
