✅ About the FacilityAhava Therapy Services is seeking a Full-Time Physical Therapist to join our in-house rehab team at Peachtree Centre, a 151-bed skilled nursing facility with 30 short-term rehab beds.
The facility offers comprehensive short- and long-term rehabilitation, as well as outpatient therapy, all supported by a state-of-the-art rehab gym designed to help residents regain independence through community reintegration and functional recovery.

Review resident information and evaluate its impact on assessment and treatment.
Conduct and document thorough functional assessments using standardized procedures.
Develop and implement individualized treatment plans with achievable goals.
Comply with evaluation, treatment, and documentation guidelines; maintain timely, accurate records.
Instruct and supervise therapy assistants as needed.
Enter treatment and progress notes for each session.
Revise treatment plans as needed throughout the course of care.
Document treatment in clear, concise discharge summaries.
Orient residents to services and treatment procedures appropriate to their needs.
Collaborate with nursing staff, residents, and families to maximize functional abilities.
Educate residents and caregivers on home exercise and therapy continuation.
Assist with referrals for additional services as needed and maintain knowledge of community resources.
Participate in rehabilitation team conferences and other facility meetings.
Access continuing education opportunities to maintain clinical competency.
Participate in quality improvement initiatives.
Follow regulatory, professional, and facility standards.
Perform other duties as assigned to provide quality care.
Understand payer reimbursement and allocate therapy resources efficiently.
Support Ahava’s mission and project a professional image.
Maintain expected productivity levels.
Bachelor’s or Master’s degree in Physical Therapy
Current PT license in the state of practice
BLS/CPR certification (current or obtained during orientation)
Strong communication, teamwork, and organizational skills
