Job Description

The Case Manager & Restorative Facilitator provides holistic, family-centered support
services designed to strengthen relationships, improve well-being, and build community
resilience. This position works with the entire family providing case management, and
restorative practices. Services include restorative circles, and connection to resources that
address social, emotional, and practical needs as well as community-based family events.
The role emphasizes trust-building, empowerment, and culturally responsive engagement
to reduce crises, increase family stability, and foster healing by supporting families in the
Department of Family Services.

ESSENTIAL FUNCTIONS:
Direct Family Services – 40%
• Facilitate intakes and needs assessments with the entire family, identifying
strengths, challenges, and goals collaboratively.
• Co-create family service plans that incorporate emotional, relational, and practical
support.
Case Management – 25%
• Connect families to community resources such as housing, food security,
employment, childcare, education, and healthcare.
• Collaborate with other service providers, schools, and community partners to
provide wraparound support.
• Maintain accurate, timely case notes and data entry in Salesforce or other systems.
• Support YCCM referred families with case management and other families referred
to other family services programs in collaboration with community partners
Restorative Practices – 15%
• Plan and facilitate restorative circles for families to address conflict, rebuild trust,
and strengthen communication.
• Host community-building circles for families to share stories, celebrate successes,
and support one another.
• Provide conflict mediation and follow-up coaching for families seeking to improve
relationships.
Family Engagement Events – 5%
• Organize family-focused events that foster connection, joy, and community
engagement (e.g., family dinners).
• Collaborate with community partners to co-host events that address family needs
and interests.
Aftercare & Follow-Up – 10%
• Conduct 30- and 60-day post-service check-ins with families to monitor progress
and address emerging needs.
• Adjust service plans based on family feedback and changing circumstances.
Transportation – 5%
• Transport families or individual members to appointments, events, or restorative
circles when needed and feasible.

QUALIFICATIONS:
Education & Experience
• Bachelor’s degree in Social Work, Psychology, Human Services, or related field
required; Master’s preferred.
• Minimum three years of experience working with families impacted by trauma,
crisis, or systemic challenges.
• Experience facilitating restorative circles or conflict mediation strongly preferred.
